Welcome to the Improving Care For Older People (IC4OP)
The Department of Human Services identified the need for and supported the establishment of centres of excellence for promoting health independence among older Victorians in each of its regions. The larger providers of integrated hospital and community health services in Hume region have agreed to work together over the next two years through a consortium arrangement to promote health independence for older people across the region as identified in the DHS Policy Framework Improving Care for Older people 2003.
The role of the consortium is to refocus culture in the health services across the Hume Region towards improving care for older people. Older people and people who have multiple and complex needs often require access to specialist assessment and treatment and a variety of support services to be able to regain or maintain their independence in the community.
The Consortium will be also produce a Quarterly Newsletter.
