IC4OP Best Practice
Health Care professionals must be aware of and understand the specific care issues for older people and how to manage these issues in accordance with practice based on BEST practice if improvement in patient care and outcomes are to be achieved.
Person centred care – putting the patient and their carer at the centre of and involving them in care planning plus decision making is a lever aspect of best practice and improving care for older people.
Knowledge and information are the key elements to ensuring the building blocks to progress and change are in place. Through IC4OP in Hume region this will be achieved in a variety of approaches:
- Publications Guidelines information to highlight innovation and promote ideas/experience sharing across the region.
- Enhancing Practice training for Managers and staff at each site run by Northern Health & COTA . This programme has been designed to promote culture change in organisations in relation to aged care focus.
Better practice in the care of older people will be strengthened through these initiatives and through the up-take of best practice evidence and the translation into daily practice and interventions the related up skilling/training of staff.
Through 2006 the Improving Care For Older People(IC4OP) will identify specific topics and care areas on which to concentrate further local research and guidance. Identified topics will become part of the strategic workplan for 2007.
