Building healthy communities in Mitchell and Murrindindi Shires

The Lower Hume Primary Care Partnership (LHPCP) is a voluntary alliance of member health agencies in Mitchell and Murrindindi Shires, Victoria, Australia.

We encourage member agencies to work together to develop a stronger, more integrated community based health and community service sector.

The Primary Care Partnership Strategy aims to improve the overall health and wellbeing of Victorians by:

• Improving the experience and outcomes for people who use primary care services.
• Reducing the preventable use of hospital, medical and residential services through a greater emphasis on health promotion programs and by responding to the early signs of disease and/or people's need for support.

Our programs are driven by the current community health plan for the catchment. This plan discribes the activities under each program that we are funded to diliver:
