The Vision for Community Health Plans
In the original Primary Care Partnership document: Primary Care Partnerships: Going Forward, April 2000, it was outlined how Primary Care Partnerships would work with the community and DHS to develop and implement a Community Health Plan (CHP). The CHP was to identify the priority health needs of the local area and describe how the providers in the Primary Care Partnership would work with each other and other key stakeholders to respond to these needs.
The Lower Hume PCP Community Health Plan
The Lower Hume PCP CHP is an operational plan that establishes goals, objectives, service system change strategies, monitoring and accountability for the ongoing implementation of the Victorian primary care reform process in Lower Hume.
The Audience
The primary audience for the CHP is The Lower Hume Forum / PCP and participating agencies. A secondary audience is Local and State government, in particular DHS – both the Central and Hume Regional offices. The information contained in this document assists these bodies to make informed decisions about health priorities and resource allocation in Lower Hume.
The plan has been written using ‘everyday’ language, making it simple and easy to read – by both health professionals and the broader community.
Community Health Plan - Planning Cycle
In the original documentation CHPs were developed based on a single financial year. Over time this has been extended to reflect the move to align the plans with other significant planning cycles at a state and local government level.
- For the years 2001 - 2004 the planning cycle was yearly, encompassing a financial year (eg July 2001 - June 2002)
- In 2004 the planning cycle was increase to two years (eg July 2004 - June 2005).
- From 2006 the planning cycle will be extend to three years (eg 2006 - 2009).
Preparation for the 2006 - 2009 CHP will commence in early 2006, with a completion date of October 2006. A key component of the process will be the identification of health and service provision priorities in the Lower Hume catchment and the outlining of an integrated response.
