LHPCP Reporting
Part of the funding agreement between Primary Care Partnerships and The Department of Human services is the requirement for periodic reporting on the implementation of the Community Health Plan.
The general reporting requirements and additional reporting requests are set out in a document called the Community Health Plan Implementation Agreement or CHPIA. The CHPIA is an agreement between the PCP and DHS that outlines what the alliance will achieve in the reporting period.
The reporting periods have changed since the initial community health plan was implemented however the most common reporting occurs mid implementation and at the completion of the plan.
