All members of the community can access the varied services available through the Primary Health Team.

Where are we?
Allied Health, Community Health services and Beechworth Planned Activity Group are located in the front building at 52 Sydney Road, Beechworth.Some services providing assessments at home (Occupational Therapy) or in community settings (Youth Counseling).
Yackanadandah Planned Activity Group is located on Monday’s and Thursday at the Senior Citizens Centre,
Wellsford Street , Yackandandah and
Wednesday at Yackatoon Village Hall,
Butson St
,Yackandandah.
No referral needed.
The Primary Health Team Services are all accessible through what is called a self referral. That means no GP referral is required.

Administrative fees
A small administrative fee is changed for some services provided by the Primary Care Team, however service will not be refused due to an inability to pay this fee.Please contact us if you have any queries and we can assist you.
