Lower Hume Primary Care Partnership


Working together towards better health and wellbeing
in Mitchell and Murrindindi communities

Located in the DHS Hume region, one hour north from the outskirts of Melbourne, Lower Hume PCP encompasses the Mitchell and Murrindindi Shire Council areas.

Lower Hume PCP is one of 31 voluntary health alliances in Victoria funded by the Department of Human Services (DHS).  Based on the Social Model of Health these alliances acknowledge that improvements in health and wellbeing are only achieved by addressing social, cultural, environmental, biological, political and economic determinants of health.
  
Our local alliance incorporates 14 health and community agencies, from hospitals to community health, family services to local government and more.
