A number of groups and committees oversee and deliver the outcomes for the Upper Hume PCP. The General Committee is the governing group of the PCP and is made up of member agencies.
The Upper Hume PCP has identified four main platforms and three working groups that will be used as avenues for activities such as strategic and service planning, consultations, lobbying, service remodelling, funding applications, resource allocation and research. These groups are broader than the PCP membership.
Platforms - are groups based around shared community interests and service delivery for specific age cohorts
Working groups - are identified to deliver specific outcomes across our membership and the broader service sector and community
The Upper Hume Healthy Communties Plan 2006-2009 is the planning document that provides an overview and key directions and goals for cooperation, collaboration and integration of work for the PCP and it's member organisations. This provides sustainability and support for delivering our health and wellbeing outcomes in our communities.
