Effective coordination among primary care services is increasingly important to service users and government alike. This group works at creating effective systemic links between primary care providers in Primary Care Parterships, based on common practices, to assist people to gain access to those services.
In summary, this group works towards:
- A system that better coordinates services in the catchment
- A consistent approach to collection and sharing of consumer information
- A process that informs further assessment
- Streamlined referral across the platforms.
The Upper and Central Hume PCPs undertook work to identify practice and establish agreement about the roles and responsibilites of organisations taking a lead or key worker role in the care of complex clients. Two reports were produced to inform this work.
